Job Description

The Cardio metabolic Clinic (CMC) RN Coordinator will be responsible for the provision and oversight of professional nursing care and medical care prescribed by the Licensed Healthcare Providers (Physician, APP, or Pharmacist) for the Cardio metabolic Clinic. Assists in facilitating care coordination of patients to ensure positive outcomes and an effective overall plan of care in a professional, confidential, and caring manner. Involved in clinical activities, working closely with providers, clinical staff, and community resources. Connects the CMC patient/family with resources for education, support and clinical needs throughout the course of treatment. Assists in data collection for the CMC Registry.

Accountable for having knowledge of and complying with the Nebraska Nurse Practice Act and other relevant State Nurse Practice Acts, any applicable licensure/certification requirements, as well as, organizational policies and procedures.

PRINCIPAL JOB FUNCTIONS:

1. *Commits to the mission, vision, beliefs and consistently demonstrates our core values.

2. *Collaborates with physicians from multi-specialties in developing referrals, coordinating clinic and outpatient visits, and appropriately navigating clinical care.

3. *Provides procedural coordination to the applicable BH Program: LAAO, Structural Heart Program, etc., and other related procedures or diagnostic testing.

4. *Receives physician referrals and efficiently compiles clinical data for presentation to a multi-specialty physician team at scheduled program meetings.

5. *Performs clinical and social-economical assessment of referred patients and navigates care between physicians and diagnostic services.

6. *Educates patient and family about tests/procedures, results, medications, nutrition and other health related issues.

7. *Performs nursing care that requires professional knowledge and judgement for assigned providers, patient follow up, patient teaching and test result communication.

8. *Triages and responds to calls from patients, pharmacies, hospitals, nursing homes, and other medical offices by use of telephone and computer, in a timely manner.

9. *Prepares Electronic Health Record (EHR) for patient visit, ensuring all necessary information is in the chart prior to appointment for multi-specialty providers, as well as, documenting patient interactions, orders, results, patient questions/concerns and medication changes, in a timely manner.

10. *Responds appropriately to urgent/emergent situations and documents as necessary.

11. *Assists with rooming; including vital signs, patient history, preparing patient for examination/procedures, performing EKG, as well as, keeping rooms stocked, clean and ready for use.

12. *Provides effective follow-up with program-specific patients per mandatory registry guidelines.

13. Maintains communication with device representatives as applicable to program.

14. Actively engages with marketing to enhance referrals and provide community education.

15. *Complies with HIPAA and maintains confidentiality in matters related to patient, family, staff, and providers.

16. *Maintains compliance with OSHA and safety plan.

17. Notifies direct supervisor of problems/incidents related to staff or patients in a timely manner.

18. Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.

19. Participates in meetings, committees and department projects as assigned.

20. Performs other related projects and duties as assigned.

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:

1. Maintains clinical competency as required for Registered Nurse licensure in State(s) of practice.

2. Knowledge of federal and state regulations related to nursing practice, OSHA and HIPAA requirements.

3. Advanced knowledge of cardiovascular clinical procedures and diagnostics.

4. Knowledge of cardiovascular rhythm recognition and cardiopulmonary monitoring assessment.

5. Knowledge of diagnosis allowed for ordering tests/procedures to support appropriate orders and avoid unnecessary denials and delays in care.

6. Knowledge of process improvement and change management principles within the healthcare environment.

7. Knowledge of computer hardware equipment and software applications relevant to work functions.

8. Ability to determine(s) effective patient teaching strategies to support patient learning.

9. Skilled in critical thinking with ability to effectively organize and prioritize projects.

10. Ability to communicate effectively both verbally and in writing.

11. Ability to work in a fast-paced environment relevant to changing patient needs.

12. Ability to work independently, take direction and work collaboratively with other health team members from diverse backgrounds.

13. Ability to establish and maintain effective working relationships with all levels of personnel and medical staff.

14. Ability to problem solve and engage independent critical thinking skills.

15. Ability to maintain confidentiality relevant to sensitive information.

16. Ability to prioritize work demands and work with minimal supervision.

17. Ability to maintain regular and punctual attendance.

EDUCATION AND EXPERIENCE:

Graduation from an accredited Registered Nursing Program is required. BSN is preferred. Current Registered Nurse licensure from the State of Nebraska or approved compact state of residence as defined by the Nebraska Nurse Practice Act required. Minimum of three (3) years of relevant nursing experience is required. Clinic and cardiovascular nursing experience is preferred.

OTHER CREDENTIALS / CERTIFICATIONS:

Basic Life Support (CPR) certification required. Bryan Health recognizes American Heart Association (for healthcare professionals), American Red Cross (for healthcare professionals) and the Military Training Network.
